Neural amyotrophy

 Neural amyotrophy:- it's a condition in which sensory and motor peripheral nerve fibers are damaged 

Charcot marie tooth disease, parsonage turner syndrome, acute brachial neuritis, idiopathic brachial plexopsthy was a condition of neural amyotrophy 

Charcot Marie tooth disease is a hereditary disorder caused by mutation in genes... this mutation cause demyelination of sensory and motor peripheral nerve 

Parsonage turner syndrome or acute brachial neuritis(inflammation of brachial nerve):- it's condition of neural amyotrophy severe shoulder pain that radiates to arm neck and scapula 

Shoulder pain


Complaints:- sudden  severe pain and weakness in the shoulder last for few weeks

Ethiology:-  

★ In most cases the causes are unknown 

★viral infection 

★ autoimmune disease 

★ excessive physical exercise 

★ stress

★ post surgery 

★ genetic (mutation in the gene)

Pathogenesis 

★ The nerve to shoulder muscle is damaged

★ most common muscle involved is infraspinatus, supraspinatus and deltoid muscle innervated by subscapular and axillary nerve

★ motor changes predominantly occur over sensory changes 

★ can involve brachial plexus from C5-T1 with variable degree of weakness 

Clinical features 

★ bilateral muscle weakness of upper limb

★severe pain and muscle wasting 

★ limitations of movement of upper limb

★ winged scapula and severe shoulder pain 

Risk factors 

★ surgical procedure 

★ excessive physical exercise 

★ stress 

Differential diagnosis 

★ Radiculopathy from herniated disc

★ adhesive capsulities or frozen shoulder 

Diagnosis 

★ electromyography EMG:-  shows nerve damage, denervation in the roots, fibrillation, hour-glass findings 

★MRI reveals atrophy and fatty infiltration 

Treatment 

★ Analgesics (opioids)

★ oral steroids like prednisolone 

★ physical therapy 

★ complete recovery in 3 years
